Internal Rules

Regulating the Rights and Obligations of Patients In accordance with Act No. 372/2011 Coll., on Health Services, the Physiotherapy Clinic Rehab Liberec s.r.o., Zákopnická 360/13, Liberec 14, 46014, ID: 05016282 (hereinafter "Rehab"), hereby establishes these Internal Rules. These rules govern the provision of health services, the rights and obligations of patients, their next of kin, health professionals, other specialists, and other persons involved in health services, as well as quality and safety assessment conditions.

These rules apply to the Rehab facility at Zákopnická 360/13, Liberec 14, 46014 (hereinafter the "facility"). Full rules are available online at: www.rehali.cz/vnitrni-rad/

I. Operating Procedures

  • Identification: Upon the first visit, patients must present their health insurance card for identity verification and to sign the Informed Consent for health services and patient safety.
  • Updates: Patients are obliged to immediately report any changes in personal data, insurance providers, or contact information.
  • Minors: Minor patients must be accompanied by a legal representative. Rehab Liberec s.r.o. does not provide supervision for minors. Patients aged 15 and over may attend unaccompanied if a "Legal Representative Consent" form is signed.
  • Appointments (New Patients): If you have an FT voucher (not a K voucher), please DO NOT KNOCK. In-person scheduling is not possible to maintain the continuity of care for scheduled patients.
  • Booking Methods:
  • Privacy & Conduct: Wait for the physiotherapist's invitation to enter. We do not open doors for knocking during sessions to ensure maximum privacy.
  • Confidentiality: We guarantee complete professional secrecy and protection of patient data.
  • Transparency: Patients must inform their therapist of all facts regarding their health status and must not withhold information.
  • Safety: Weapons, explosives, and flammable substances are prohibited. Smoking, e-cigarettes, alcohol, and drug use are strictly forbidden on the premises.
  • Electronic Devices: Silence your phone in the waiting room and clinic. All sounds must be turned off before entering the treatment room.
  • Emergency: In case of fire or other emergencies, follow staff instructions and use designated emergency exits.
  • Liability: Rehab Liberec s.r.o. is not responsible for items left in the waiting room.
  • Capacity: Physiotherapy via FT voucher is generally limited to once per year. Multiple instances of care per year are only possible in specific cases.
  • Payments: For self-payers or services not covered by insurance, see the price list at www.rehali.cz/cenik/. We accept cash or QR code transfers.

II. Legislation Regarding FT Vouchers

  • Scheduling Deadline: The voucher is valid for scheduling for 14 days from the date of issue.
  • Start Date: Therapy must begin within 40 days of the voucher's issuance. If our capacity does not allow for this, the client must seek another facility.
  • Expiration: Therapy covered by an FT voucher must be completed within 5 months of its issuance.
  • Confirmation: Clients have 3 days to confirm scheduled dates. Unconfirmed dates will be offered to other applicants.
  • Cancellations: If canceled in time, a replacement session is only possible if the facility has capacity before the voucher expires (5 months).
  • Rejection: An FT voucher may be refused if it does not meet legislative requirements for insurance reimbursement.

III. Patient Obligations

  • Documentation: At the first session, present your insurance card, the original FT voucher, and any relevant medical reports.
  • Cancellations: If you cannot attend, you must apologize at least 24 hours in advance, preferably via SMS or email. For same-day cancellations, a replacement session cannot be guaranteed.
  • Reminders: If a phone number is provided, a reminder SMS is sent 48 hours before the appointment. Failure to cancel is considered a gross violation of the Internal Rules.
  • Consequences of No-Shows: Unexcused absences may result in the immediate termination of treatment without compensation and removal from care. The facility reserves the right to cancel all remaining appointments.
  • Illness: If you have a viral/bacterial infection, fever, or are taking antibiotics, you must reschedule.
  • Contraindications: Patients showing signs of acute infection, respiratory illness, or intoxication will not be admitted.
  • Hygiene: Patients must maintain proper personal hygiene and wear clean clothes. Please wash or disinfect your hands before entering the clinic.
  • Professionalism: Treat medical staff and other clients with respect and follow staff instructions.

IV. Patient Rights

According to Act No. 372/2011 Coll., you have the right to:

  • Consent: Health services are provided only with your free consent (implied by booking and submitting a voucher).
  • Quality: Services provided at a professional standard with dignity and respect for privacy.
  • Choice: The right to choose a provider authorized to provide the necessary health services.
  • Information: To be informed of the names of staff, the price of non-covered services, and your health status/treatment plan.
  • Privacy: To refuse the presence of persons not directly involved in your care.
  • Support: The continuous presence of a legal representative or designated person, provided it does not disrupt care.
  • Accessibility: To communicate in a manner understandable to you (including interpreters, except for Slovak). Assistance dogs are permitted for those with sensory or physical disabilities.
  • Documentation: To inspect your medical records and request copies (within 30 days of a request).
  • Confidentiality: To prohibit the disclosure of your health information to any person.

Final Notice: The Internal Rules are binding for all clients. Disagreement with these rules constitutes a refusal of care at Rehab Liberec s.r.o. In such cases, you may choose a different clinic. A gross violation of these rules is grounds for refusal of treatment or removal from our care.
